(a) state the name of the court;(b) state the names of the parties;(c) state the date, time, and location of the hearing;(d) state that the parties shall appear at the hearing, unless otherwise provided in these rules;(e) inform the parties of the requirement to bring to the hearing sufficient copies of all documents the parties intend to offer; and(f) if possible, include the name of the child support magistrate assigned to the case.
