An order issued in compliance with section 3113.451 of the Revised Code shall include the following:
(A) The name and billing telephone number of the account holder; (B) The name and contact information of the petitioner to whom the wireless service number or numbers shall be transferred; (C) Each wireless service number to be transferred to the petitioner.
