The Unified Judicial System forms and the sample forms contained in the Appendix of Forms to these rules are provided as general examples for use in the Unified Judicial System and should be modified to comport with the particular circumstances of each case; they have not been approved or adopted by the Alabama Supreme Court as officially approved forms.

Committee Comment
Rule 36 is intended to inform the practitioner that strict compliance with the Appendix of Forms is not necessarily a prerequisite for complying with the rules. The forms are intended as aids to practice only and may be modified according to the requirements of a particular case.
Court Comment
The forms appearing in the Appendix of Forms are those that were included when the Court amended Rule 36 effective January 10, 2001. Those forms had not been approved by the Supreme Court as “officially approved forms.” See Rule 36. The revision date shown on each Unified Judicial System form indicates the date the Uniform Judicial System’s Forms Committee last updated that form. The sample forms are those that were included when the Supreme Court adopted the Alabama Rules of Criminal Procedure on May 31, 1990, and that have not been converted to Uniform Judicial System forms. The Unified Judicial System’s Forms Committee may from time to time draft additional forms, some of them to replace forms included in the Appendix of Forms. These new forms have been distributed to circuit court clerks and district court clerks for inclusion in the “Court Forms Manual.” Many of these forms, as periodically updated, may be found at the Administrative Office of Courts’ Web site: www.alacourt.org/, under the heading “UJS Court Forms.”
